After downloading Adobe, select your tax forms from the drop-down boxes below. All Federal Tax Forms listed can be completed on-line.

Ohio Statement of Non-Resident form

To begin or discontinue resident tax withholdings, click on Local Tax Forms.

Applicable to Michigan, Ohio and Pennsylvania resident tax only.

To obtain an exemption from the Local Services Tax in PA for the current year, use this form.

If you need to request a reprint of your W-2 form, please call HR InfoLine at 1-888-881-1121 and speak with a Customer Service Representative. You will need your HR InfoLine PIN to make this request. If you do not remember your PIN, please stay on the line and the Customer Service Representative will assist you.

After completing your local, state or federal (W-4) tax withholding forms, you should return them to HR Payroll Tax Services at locator #01-6134.
